Recruitment stages
Stage I: Electronic registration – filling out the recruitment form electronically.
Stage II: Submission of documents and collecting the referral for a medical examination
Recruitment for 7-semester engineering studies in Information technology takes place in two stages:
Stage I
Online registration – filling in the recruitment form in the WSTI electronic recruitment system.
Stage II
Submission of documents and collecting referral for a medical examination - the candidate is required to obtain the referral for a medical examination and shall submit the required documents:

photocopy of the Secondary School Final Examination results and the original available for inspection (IMPORTANT! A copy of the Secondary School Final Examination results will be treated as an original document);

photocopy of identity card (both sides) - original for inspection,
-
2 recent color photos (35mm x 45mm without a hat on a light background)

medical certificate issued by the occupational medicine physician stating no contraindications to undertake studies in Information technology (download the form: *.pdf)
– referral for the medical examination to be collected from the Recruitment Office and after obtaining a medical certificate submit a set of documents,
– at the Recruitment Office one can submit all documents except the medical certificate, then get a referral for medical examination and conduct it at the office of an occupational medicine physician - the medical certificate must be delivered in person or sent by post within two weeks from the date of submission of documents and collecting the referral, -
proof of payment of the administrative recruitment fee (does not apply to candidates during the promotion period)

certificate of disability, if a degree of disability was determined.

Declaration of will to study at the Katowice Institute of Information Technologies (please read the document). The Declaration will be issued for signature at the Office of Recruitment.

Educational Agreement. The document will be signed together with the decision on admission for studies.
Within 14 days from the date of registration in the online registration system (stage I) the candidate is obliged to personally deliver the set of documents referred to above, as well as submit the results of the medical examination and sign a declaration of will to study and personal information questionnaire.
Remember to sign up for a visit to the occupational medicine physician, and then collect a referral in our Recruitment Office. Tests may be performed at the PROFIMED – ul. Gliwicka 2, Katowice – after prior registration by phone or mail.
Foreigners are required to provide:

A photocopy of a visa, residence card or other document entitling to stay in the territory of the Republic of Poland and the original for inspection; a photocopy of the Pole's Card and the original for inspection (if applicable).

A photocopy of the Maturity Certificate or a document equivalent to the Polish Maturity Certificate, translated by a sworn translator entered on the list of sworn translators of the Ministry of Justice of the Republic of Poland and bearing an apostille clause, and the original for inspection.

Certificate confirming the knowledge of the Polish language at the language proficiency level of at least B2, issued by: the State Commission for the Certification of Proficiency in Polish as a Foreign Language, authorized by the Minister responsible for higher education and science to organize examinations in Polish as a foreign language at a certain language proficiency level (a Polish or foreign university conducting studies in Polish philology, a Polish or foreign entity that has been conducting foreign language courses or classes in teaching Polish or Polish as a foreign language or other didactic classes in Polish for at least three years).

A photocopy of the insurance policy in case of illness or accident, covering the period of education in Poland, and the original available for inspection. All documents issued in a foreign language will be accepted together with a translation into Polish prepared by a sworn translator entered on the list of sworn translators of the Ministry of Justice of the Republic of Poland.
DELIVERY OF DOCUMENTS IS A NECESSARY CONDITION WHEN REGISTERING FOR STUDIES!

Medical examination
Referral for examination must be collected at the Recruitment Office. Tests may be performed at the TOMMED/PROF-MED, ul. Żelazna 1, 40-952 Katowice – after prior registration by phone or mail.
CANDIDATES TAKE NOTE!!!
Referral for medical examination is to be collected at the Recruitment Office (Institute Dean Office).
Tests conducted at the TOMMED/PROF-MED are reimbursed by the Institute.
Medical certificates (on the required document, issued by a doctor authorized to carry out preventive examinations) made in other occupational health clinics will be accepted.
